Job description

Full-Time Physical Therapist | Memory Care | Practice Therapy Where Life Happens
Memory Care | Full-Time
What if physical therapy wasn't just about helping someone move better?
What if it was about helping them continue to live their life?
At Prava, we believe therapy in Memory Care requires more than a traditional treatment plan and a therapy room.
It requires knowing the person.
Understanding the environment.
Recognizing the routines that matter.
And finding ways to help someone participate—even when dementia has changed what independence looks like.
We're looking for a Full-Time Physical Therapist who wants to practice that way.
We don't just work bankers' hours.
Because residents don't live bankers' hours.
Sometimes the most important mobility intervention isn't a walk down a hallway at 10 a. m.
It's getting safely to dinner.
Getting to an activity.
Navigating a transition.
Practicing a transfer during an actual routine.
Helping someone move through their environment with more confidence.
We go where the function is.
We don't expect the resident to fit the therapy.
We adapt the therapy to the resident.
Someone living with dementia may not be able to explain why walking has become difficult.
Maybe the environment is overwhelming.
Maybe the routine has changed.
Maybe the cueing isn't working.
Maybe the task needs to be approached differently.
Maybe the environment—not the resident—is creating the barrier.
Your job isn't simply to make the resident perform better in therapy.
Your job is to figure out how to help them function better in their world.
Your role
As a PT, you'll provide skilled, function-focused care to residents living with dementia.
You'll have the opportunity to:
Evaluate and develop individualized plans of care
Address gait, balance, transfers, strength, endurance, mobility, and fall risk
Practice functional mobility in the environments where residents actually live
Incorporate real-world routines and meaningful activities into treatment
Adapt interventions based on cognition, communication, behavior, and environment
Identify environmental barriers and collaborate on solutions
Partner with caregivers and community staff to support carryover
Help residents maintain mobility, participation, dignity, and quality of life
Build relationships with residents and learn the person behind the diagnosis
We believe Memory Care therapy starts with the person.
Not the diagnosis.
Not the behavior.
Not the impairment.
The person.
What matters to them?
What routines are familiar?
Where do they feel successful?
What helps them feel safe?
What is getting in the way of participation?
What can we change?
Those questions should shape the therapy.
We're looking for a PT who:
Loves working with older adults living with dementia
Believes mobility is about participation—not just distance
Understands the importance of environment, routine, and meaningful engagement
Is patient, observant, creative, and adaptable
Builds relationships naturally
Is comfortable collaborating closely with caregivers and community teams
Wants to practice clinically rather than simply follow a formula
Doesn't believe every therapy session needs to happen in a therapy room
If you've ever thought, “There has to be a better way to do therapy in Memory Care,” you'll probably like Prava.
Because we don't just want to make someone a better therapy patient.
We want to help them continue being the person they are.
And sometimes the most important therapy of the day happens after 5 p. m.
Come practice PT where life happens.
Come practice Prava.
