Key Responsibilities
The physical therapist will conduct home visits to LIFE program participants to evaluate, develop, and implement personalized treatment plans. They will coordinate care with an interdisciplinary team and ensure accurate, timely documentation of patient outcomes and billing records.
Requirements
Candidates must have graduated from an accredited physical therapy school and hold a valid New Jersey Physical Therapist license. One year of experience in acute adult inpatient, outpatient, or pediatric physical therapy is preferred.
