Independence Home Health LLC is looking for a compassionate, dependable, and motivated Registered Nurse (RN) Case Manager to join our growing home health team!

What You'll Do
- Conduct patient assessments and develop individualized plans of care.
- Coordinate care with physicians, patients, families, and other healthcare professionals.
- Educate patients and caregivers on care plans, treatments, and ongoing health needs.
- Supervise and support LPNs, HHAs, and CNAs to promote quality patient care.
- Review and update plans of care as needed.
- Maintain accurate, timely, and complete clinical documentation.
- Participate in case conferences and quality improvement initiatives.
- Help ensure patients receive safe, compassionate, and effective care in their homes.
What We're Looking For
- Graduate of an accredited nursing program.
- Active Indiana RN license or qualifying multistate license recognized by Indiana.
- Minimum of 2 years of nursing experience; home health experience preferred.
- Current CPR certification.
- Strong clinical, communication, leadership, and organizational skills.
- Reliable transportation and willingness to travel to patients' homes as needed.
- A caring, patient-centered approach and a genuine desire to make a difference.

Key Responsibilities
The Registered Nurse Case Manager will conduct patient assessments, develop individualized care plans, and coordinate care with physicians and families. They are also responsible for supervising LPNs, HHAs, and CNAs while maintaining accurate clinical documentation.
Requirements
Candidates must be graduates of an accredited nursing program with an active Indiana RN license and at least two years of nursing experience. A current CPR certification and the ability to travel to patient homes are also required.
