Social Worker

Fox Subacute - Mechanicsburg is seeking a dedicated and compassionate Social Worker to join our team in Mechanicsburg, PA. This role offers an opportunity to make a meaningful impact on the lives of our patients by providing emotional support, resource coordination, and advocacy within a subacute care setting. The Social Worker will collaborate closely with multidisciplinary teams to ensure comprehensive care and support for our patients and their families.

Responsibilities

• Conduct psycho-social assessments of patients and their families to identify needs and provide appropriate support.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Advocate for patient rights and access to community resources.
• Provide counseling, crisis intervention, and emotional support to patients and families.
• Coordinate discharge planning and facilitate referrals to community services and support networks.
• Maintain accurate documentation and comply with all regulatory and facility policies.

Requirements

• Bachelor’s or Master’s degree in Social Work from an accredited program.
• Valid license or certification as required by the State of Pennsylvania.
• Previous experience in a healthcare or subacute care setting preferred.
• Strong communication, interpersonal, and problem-solving skills.
• Ability to work effectively both independently and as part of a multidisciplinary team.
