Key Responsibilities
The Licensed Social Worker oversees the planning, implementation, and direction of the facility's Social Services Department in compliance with federal and state standards. They also manage initial contact with discharge planners and families to facilitate facility tours and follow-up procedures.
Requirements
Candidates must hold a current Social Work license in good standing and possess at least 3 years of experience in a long-term care facility. A Bachelor’s Degree in Social Work is strongly preferred.
