What You’ll Do
- Evaluate patients and develop individualized therapy plans
- Provide in-home physical therapy treatments
- Improve mobility, strength, balance, and functional independence
- Educate patients and caregivers on exercises and safety techniques
- Monitor progress and adjust treatment plans as needed
- Complete accurate and timely documentation
What We’re Looking For
- Active Texas Physical Therapist (PT) license (required)
- Home health experience preferred
- Strong clinical judgment and patient care skills
- Ability to work independently in a home setting
- Excellent communication and documentation skills
- Reliable transportation & valid driver’s license
- Kinnser/WellSky EMR experience (preferred)

Requirements
Candidates must hold an active Texas Physical Therapist (PT) license and possess reliable transportation with a valid driver's license. Experience in home health and familiarity with Kinnser/WellSky EMR systems are preferred.
