Manage a panel of high-risk patients with uncontrolled chronic diseases through direct face-to-face care and medication optimization. Oversee pharmaceutical care, design therapy regimens, and collaborate with multidisciplinary healthcare teams to improve patient outcomes.
Requires a PharmD or Bachelor's in Pharmacy with a PGY1 residency minimum; PGY2 in ambulatory or cardio is strongly preferred. Must be licensed in the state of practice and obtain board certification (BCPS or BCACP) within one year.
