About the Role

Locations: Career High Schooland Church/Helen Street.

We are seeking an experienced Pediatric or Family Practice. Due to the expansion of services, Cornell Scott Hill Health Corporation, a leader in health services, is seeking an experienced APRN to join our team! This is an excellent opportunity to work within a nationally recognized Federally Qualified Health Care Center where treating the entire person is at our forefront.

Cornell Scott Hill Health Center is in need of an Advanced Practice Registered Nurses (APRN) to work in our School Based Health Clinics (SBHC). The APRN practices medicine under a collaborative agreement with a departmental physician, with both disciplines practicing within the scope and boundaries of State regulations. Provides reasonable and prudent medical/surgical care, practices appropriate management, and uses sound clinical judgment based on best practice standards and evidenced based medicine. Provides general medical care, patient education and treatment to patients of CS-HHC.  

Key Responsibilities

* Coordinates patient care with colleagues, nursing, front desk outreach, case management staff and other agencies as necessary

* Educates patients on preventive health care and chronic disease management

* Protects clients and employees by adhering to infection-control policies and protocols

* Completes charting of notes, forms and coding in a timely and efficient manner. Enters appropriate legible notations, documents encounters appropriately

* Maintains confidentiality in regards to patients, staff and program matters at all times

Required Qualifications

* 3+ years of APRN experience within a healthcare facility

* MSN degree and current CT RN/APRN licensure.  CPR certification required

* Advanced knowledge of EPIC EMRs

* Willingness to work a flexible schedule and to fill in when needed

* Knowledge and management of principles and practices including optimum use of staff and all other available resources.

* Ability to explain treatments and conditions to patients of all ages

* Working knowledge of health care laws and regulations

Travel within the State of Connecticut is required on an occasional basis.

Requirements

Candidates must hold an MSN degree, current CT RN/APRN licensure, and CPR certification. A minimum of 3 years of APRN experience and proficiency with EPIC EMR systems are required.
