Key Responsibilities

The RN Case Manager will develop and implement individualized care plans while providing compassionate, one-on-one support to hospice patients and their families. They are responsible for conducting clinical assessments, administering medications, and collaborating with interdisciplinary teams to ensure high-quality end-of-life care.

Requirements

Candidates must hold a valid Registered Nurse license in good standing and a current CPR certification. A valid driver's license and reliable transportation are required to perform home visits.
