Key Responsibilities
The social worker provides advanced clinical services, care planning, and case management for ALS patients and their families. They also conduct assessments, coordinate community resources, and participate in interdisciplinary team consultations and quality improvement initiatives.
Requirements
A Master's degree in Social Work is required for this position. Candidates with NYS LMSW or LCSW licensure and at least one year of experience in a medical, psychiatric, or healthcare setting are preferred.
