If you're a Family Nurse Practitioner or Physician Assistant who wants to practice full-scope primary care with genuine clinical autonomy, we'd love to talk.
What You'll Do
You'll serve as the primary clinician for a defined patient panel, supported by a dedicated Medical Assistant and a collaborating physician.
Provide longitudinal primary care: preventive care, chronic disease management, and acute visits
Manage chronic conditions (diabetes, hypertension, hyperlipidemia, and more) with the panel size and visit time needed to intervene before conditions progress
Perform in-office procedures including I&Ds, suturing and suture removal, skin biopsies, and men's and women's health exams
See patients in person, by phone, and via secure video/text — typical visits run 30 minutes to 1 hour depending on patient need
Direct daily patient care and delegate tasks to your assigned Medical Assistant
Document visits and maintain records in our EHR
Provide after-hours patient access through a company-provided secure line (your personal number is never shared with patients)
Practice under a collaborating/delegating physician in accordance with Texas Medical Board requirements, with mentorship from our Medical Director team
Participate in weekly team meetings and 1:1s
What You'll Bring
Required:
Active, unencumbered Texas licensure as an APRN-FNP (national board certified) or PA (NCCPA-certified)
2+ years post-graduate experience in family practice or primary care
Experience managing patients across the lifespan (pediatric through geriatric)
Hands-on procedural experience: I&Ds, suturing/suture removal, skin biopsies, IV placement, men's/women's health exams
Active, unrestricted DEA registration (or ability to obtain before start)
Current BLS certification (or ability to obtain within 30 days)
Comfort with electronic medical records and scheduling tools
Preferred:
Prior experience in direct primary care, concierge medicine, or employer-sponsored clinics
Bilingual (English/Spanish)
Experience owning a patient panel with a high degree of clinical autonomy
Telehealth experience or multistate licensure
A preference for lower-volume, higher-touch medicine over high-throughput fee-for-service

Requirements

Requires active Texas licensure as an APRN-FNP or PA with at least 2 years of post-graduate experience in family or primary care. Candidates must possess hands-on procedural skills and maintain active DEA registration and BLS certification.
