Job Description

  • Provide medically necessary and clinically appropriate services to identified clients with a serious mental health diagnosis and an emotional, cognitive, and/or behavioral impairment that is a direct result of the mental illness.
  • Utilize the designated evidence-based model(s) for delivering Rehabilitative Mental Health treatment.
  • Restore the functioning of adults with mental health diagnosis to the fullest extent possible with the least amount of professional intervention and to minimize the disabling effects on the individual’s capacity for community living and/or limit the periods of inpatient treatment.
  • The overarching focus is to utilize the personal strengths, resources, and natural supports to reduce functional deficits associated with their mental illness and increase restoration of independent functioning.
  • Provide face-to-face services primarily at CrescentCare locations, or in the community when medically necessary to achieve a component of the treatment plan.
  • Works as part of the integrated medical and behavioral health care team within the agency and the community at large to serve clients and improve health outcomes. POSITION REQUIREMENTS Education (required)
  • A master’s degree in either social work or a CACREP-accredited graduate program. Licensure (minimum required and maximum)
  • Minimum: Provisionally Licensed Professional Counselor (PLPC), Licensed Master Social Worker (LMSW), or Provisionally Licensed Marriage and Family Therapist (PLMFT).
  • Maximum: Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT). Other
  • Minimum age of 21.
  • Must have reliable transportation and a valid driver’s license, no license denials, revocations, or more than one suspension in the past three years; valid car insurance; no more than one at-fault accident in the past three years and no more than two moving violations in the past five years.
