The Social Worker will resolve psycho-social barriers for patients and support their discharge needs within a collaborative care management team. They will work alongside RN Case Managers and Utilization Management staff to ensure effective patient progression and appropriate care delivery.
Candidates must hold a Master’s degree in Social Work (MSW) or Master of Social Services (MSS) from an accredited school. A valid CSW or MSW license in the state of Delaware is required prior to the hire date.
