Job Description
A Brief Overview
Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves a resource to others in work-team and department.

Education
- Master's Degree from an accredited school of Social Work (Required)

Key Responsibilities

Requirements
Candidates must hold a Master's degree in Social Work and possess a Licensed Social Worker (LSW) certification. Previous experience in a hospital or health agency setting is preferred, along with the ability to obtain an LISW license within one year.
