Key Responsibilities

The Nurse Navigator coordinates patient-centered care, education, and advocacy for patients in value-based and home-based care models. They manage complex patient populations, facilitate transitions across the healthcare continuum, and address barriers to care to improve health outcomes.

Requirements

Candidates must hold a Bachelor's degree in Nursing and possess a current Ohio RN license. A minimum of two years of recent clinical experience is required, with preference given to those with backgrounds in primary care or case management.
