Key Responsibilities
Provide one-on-one physical therapy to older adults in their homes or senior living communities using the Geriatric House Calls model. Collaborate with an interdisciplinary team to develop individualized care plans and document progress within the EMR system.
Requirements
Candidates must hold a valid Physical Therapy license or be eligible to obtain one. A degree from an accredited Physical Therapy program and current CPR certification are required.
