Compensation Package

• Per Visit: $70 - $130 per completed visit (rate varies by visit type and experience)
• Paid Drive Time
• Mileage Reimbursement: $0.72 per mile
• Productivity Bonus: Earn additional incentives based on performance
• Documentation Bonus: Reward for timely submission of documentation

As a Physical Therapist (PT) with GrandCare, you'll provide in-home physical therapy services according to a physician's plan of care, helping patients regain strength, mobility, confidence, and independence following surgery and other medical conditions.

This is more than a home health position. It's an opportunity to become a trusted partner in your patients' recovery and celebrate their progress every step of the way.

Key Responsibilities

• Perform home health physical therapy evaluations and develop care plans
• Provide therapeutic treatment and instruct patients/caregivers
• Monitor progress and report changes to physicians
• Participate in discharge planning and interdisciplinary case conferences
• Supervise Physical Therapy Assistants per state regulations

Qualifications

• Current California Physical Therapy License
• 2+ years of physical therapy experience
• CPR certification
• Valid driver's license and insured vehicle
