Key Responsibilities
Provide longitudinal, holistic primary care to medically complex patients within their homes and communities. Collaborate with interdisciplinary teams to manage chronic and acute conditions while addressing social determinants of health.
Requirements
Must be a Nurse Practitioner or Physician Assistant with a master's degree and active licensure. Requires a passion for geriatric care and the ability to work independently in home-based environments.
