The RN Case Manager will deliver hands-on hospice nursing care and coordinate services for patients within the community. They will also collaborate with the clinical team to develop personalized care plans and provide support to patients and their families.
Candidates must hold a current unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Proficiency in EMR systems, a valid driver's license, and current CPR certification are also required.
