Hospice RN Case Manager

Key Responsibilities

The Hospice RN Case Manager coordinates patient care by conducting comprehensive assessments, developing plans of care, and collaborating with the interdisciplinary team. They are responsible for monitoring patient conditions, managing order changes, and providing clinical documentation in accordance with agency policies.

Requirements

Candidates must be a graduate of an accredited school of nursing and hold a current, unencumbered RN license. Experience in long-term care, homecare, or hospice is preferred, along with the ability to pass background, TB, and drug screenings.
