Key Responsibilities

The Hospice RN Case Manager provides comprehensive end-of-life care, coordinates patient-centered plans, and manages a caseload of hospice patients. They perform clinical assessments, supervise hospice aides, and collaborate with interdisciplinary teams to ensure patient comfort and regulatory compliance.

Requirements

Candidates must hold a current Registered Nurse (RN) license and possess strong clinical assessment and communication skills. At least one year of experience in hospice or a related healthcare setting is preferred, along with a valid driver's license and CPR certification.
