Registered Nurse (RN) - Case Manager

Key Responsibilities

The Registered Nurse Case Manager will own patient assessments, documentation, and the delivery of high-acuity home health care, including wound care and infusion therapy. They are responsible for coordinating care across disciplines, maintaining clear plans of care, and ensuring continuity for patients while supervising LVNs and HHAs.

Requirements

Candidates must hold a current Texas RN license and possess at least 3 years of clinical nursing experience. Proficiency in wound care, infusion therapy, and OASIS documentation is required, along with a valid driver's license for field-based work.
