Performs complex public health social work and case management services for children with special health care needs and high-risk pregnant women. Conducts home visits, develops service plans, and collaborates with community partners to address client needs.
Requires a bachelor's or graduate degree from an accredited social work program and a current, unrestricted license to practice social work in Texas. Candidates must be able to provide their own transportation for home visits and field work.
