Performs complex public health social work and case management services for children with special health care needs, high-risk pregnant women, and the general public. Conducts home visits, completes assessments, develops service plans, and collaborates with community partners to address client needs.
Requires a current, unrestricted social work license issued by the Texas State Board of Social Worker Examiners and a valid Class C Texas Driver's License. Candidates must be able to travel up to 40% of the time and provide their own transportation for field duties.
