The Case Manager coordinates and manages patient care to ensure continuity, compassion, and reliability throughout the hospital stay. They collaborate with physicians to evaluate patients and facilitate safe, timely discharge plans.
Candidates must hold a current Registered Nurse license in the State of Michigan and have at least 3 years of progressive clinical experience. A Bachelor of Science in Nursing is preferred, and Case Management certification is encouraged.
