Job Description

  • Role: Orthopedic Physical Therapist (PT)
  • Schedule: Full-time, Monday–Friday
  • Location: Field-based, Dallas
  • Compensation: $115,000–$130,000; paid per productivity point

Who We Are

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model. Our mission is to dramatically improve patient access to home health care.

As an Orthopedic Physical Therapist, you’ll help patients regain mobility, function, and confidence at home after orthopedic surgery, injury, or other musculoskeletal conditions. You’ll own the therapy plan and drive meaningful outcomes, not just complete visits.

We also build technology to support field clinicians. Our charting product reduces documentation burden, and our scheduling product will make routing and visit coordination easier—protecting your time.

What You’ll Do

Own your caseload as a case manager: Be accountable for each patient’s therapy plan, progression, and outcomes, not just individual visits.

Complete comprehensive in-home assessments and deliver skilled PT visits for patients recovering from orthopedic surgery including joint replacement, fractures, spinal procedures, and musculoskeletal injuries.

Develop and execute individualized plans of care focused on pain-limited mobility, range of motion, strengthening, gait, balance, transfers, functional mobility, home safety, and fall prevention.

Follow physician orders and applicable post-operative protocols, including weight-bearing and movement precautions; adapt treatment to each patient’s recovery and clinical presentation.

Set measurable goals, track objective progress, and adjust treatment when recovery stalls or needs change.

Recognize and escalate changes in condition or potential post-operative complications to the appropriate clinician or provider.

Coordinate with patients, caregivers, physicians, and the multidisciplinary care team to align on orders, treatment, and transitions in care.

  • Manage treatment and outcomes directly, including PTA supervision where appropriate.
  • Coach patients and caregivers on safe mobility, assistive devices, precautions, and practical home exercise programs.
  • Document promptly and thoroughly—defensible, efficient, and consistent.

Why Adaptive

Operations that back you up: You’re supported by an experienced clinical and operational team so you can focus on patient care, not chasing logistics.

Clear productivity model with upside: You’re paid per productivity point, with an expectation of 28 points per week and the opportunity to earn more for exceeding it.

Schedule flexibility: We build schedules to make field work sustainable and support smart routing and coordination, so your day is predictable whenever possible.

Technology built for clinicians: Our charting tools are designed to reduce documentation burden, and our scheduling product (launching soon) will streamline visit coordination and reduce last-minute changes.

Mission-driven work: Help expand access to high-quality home health care and raise the standard for orthopedic recovery at home.
