The Nurse Practitioner will conduct medical evaluations and provide compassionate care to elderly and seriously ill patients in their home environments. They will also collaborate with the interdisciplinary team to develop care plans, manage symptoms, and ensure accurate clinical documentation.
Candidates must hold a current, unrestricted Nurse Practitioner license in Michigan and possess excellent interpersonal and organizational skills. A minimum of one year of experience in a Medicare-certified home health or hospice agency is preferred.
