The Nurse Practitioner will conduct medical evaluations and provide palliative care to elderly and seriously ill patients in their home environments. They will also collaborate with the interdisciplinary team to coordinate care, manage symptoms, and assist families with advance care planning.
Candidates must hold a current, unrestricted Nurse Practitioner license in Michigan and possess excellent interpersonal and organizational skills. At least one year of experience in a Medicare-certified home health or hospice agency is preferred.
