The Nurse Practitioner will conduct medical evaluations and provide palliative care to patients in their home environments while collaborating with an interdisciplinary team. They are responsible for prescribing medications, coordinating care plans, and educating patients and families on disease processes and symptom management.
Candidates must hold a current, unrestricted Nurse Practitioner license in Michigan. A minimum of one year of experience in a Medicare-certified home health or hospice agency is preferred.
