Key Responsibilities

The Nurse Practitioner will provide comprehensive care to senior Medicare patients, including conducting health assessments, creating care plans, and coordinating with an integrated care team. They will also perform clinical screenings, manage chronic conditions, and ensure accurate clinical coding and documentation.

Requirements

Candidates must hold a current Utah Nurse Practitioner license, be board certified, and possess at least 2 years of clinical experience in primary care or related fields. Proficiency in chronic care management and knowledge of HCC coding are required, along with active CPR/BLS certification.
