Job description
Join Cleveland Clinic as a Social Worker - Ambulatory Outpatient Services in Cleveland, Ohio.
Key Responsibilities
- Develop plans of care for patients or populations based on current situation and assess needs of the patient, caregiver and community or the population to be served.
- Collaborate with post-acute providers to meet the needs of complex patients between ambulatory care visits.
Requirements
- Requires good visual and hearing acuity through normal corrected means.
- Requires manual dexterity to document in EHR and work with office equipment and other technology as required.
Work Schedule
- Full-Time position.
- Hybrid position - combination of remote and on-site.

Licensure & Certifications
Minimum qualifications require a Master's degree in Social Work (MSW) and current state licensure as a Social Worker (LSW, LISW, LISW-S, or PC). Candidates must also possess Basic Life Support (BLS) certification and experience in ambulatory care, hospital patient care, or a related field.
