Key Responsibilities
Providers conduct comprehensive adult and geriatric health assessments at members' homes or skilled nursing facilities to review medical history and identify health risks. They collaborate with primary care physicians, caregivers, and medical personnel to ensure effective follow-up and care management.
Requirements
Candidates must hold a Master's degree and current Nurse Practitioner licensure with board certification in a relevant specialty. At least one year of NP experience and the ability to travel extensively for home visits are required.
