Key Responsibilities
The RN Case Manager will assess patient needs, create personalized hospice care plans, and provide direct nursing care including pain and symptom management. They will also collaborate with an interdisciplinary team and maintain accurate, timely clinical documentation.
Requirements
Candidates must be graduates of an accredited nursing school with a current nursing license and at least one year of nursing experience. A valid driver's license and insured vehicle are required for travel throughout the assigned territory.
