Job Description
Community Care is hiring a RN Case Manager to join our Family Care Program team. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, being part of a mission driven organization and who shares our mission to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This is a full-time opportunity working out of our Waukesha location.

RN Case Manager Job Responsibilities
Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed.
Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services.
RN Case Manager Job Requirements
- Education: Associates degree in Nursing Required, Bachelors Degree preferred.
- Licensure: Wisconsin Licensed Registered Nurse
- Licensure: Eligibility for long-term care functional screener certification is required
- Licensure: WI Drivers License
Experience: Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity Required
Preferred 2 years of work in community nursing
RN Case Manager Job Schedule
Monday- Friday 8:00 AM-4:30 PM.

Key Responsibilities
The RN Case Manager completes initial and ongoing assessments while developing and maintaining comprehensive care plans for members. They conduct home visits and coordinate services to ensure quality and effectiveness for elderly individuals and adults with disabilities.
Requirements
Candidates must hold an Associate degree in Nursing and a valid Wisconsin Registered Nurse license. At least two years of recent experience in community nursing or working with older adults and individuals with disabilities is required.
