Key Responsibilities
Provide comprehensive psychosocial services to terminally ill patients and their families, including assessments and supportive counseling. Manage patient caseloads, participate in interdisciplinary team meetings, and assist with end-of-life planning and bereavement support.
Requirements
Requires a current Social Worker license and a minimum of two years of experience in a healthcare or social services setting. A Bachelor's degree in Social Work is required, with a Master's degree preferred, along with a valid driver's license and auto insurance.
