Key Responsibilities
The nurse case manager will provide in-home nursing and coordinate care for geriatric patients, including medication reconciliation and documentation. They will also communicate treatment plans to families and providers while ensuring continuity of care and HIPAA compliance.
Requirements
Candidates must hold an LPN or RN license and have at least 2 years of experience working with geriatric populations. Proficiency in EHR systems and phlebotomy skills are required for this role.
