Key Responsibilities
The Nurse Practitioner will conduct comprehensive in-home assessments and manage acute or chronic medical conditions for high-risk health plan beneficiaries. They will collaborate with an interdisciplinary team to provide clinical care, patient education, and advanced care planning in the patient's place of residence.
Requirements
Candidates must hold an active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care and possess an unrestricted state licensure. A valid driver's license and access to reliable transportation are required for daily travel to patient homes.
