Key Responsibilities
Assess the psychosocial needs of patients and families to develop and implement effective care plans and complex discharge strategies. Collaborate with the multidisciplinary team to ensure seamless transitions and provide expertise in social work practice.
Requirements
Requires a Master's degree in Social Science or Social Work and up to one year of clinical healthcare or community social work experience. Must be a licensed social worker with strong computer literacy and knowledge of payer reimbursement rules.
