Key Responsibilities
The Nurse Practitioner or Physician Assistant will conduct comprehensive in-home health assessments for Medicare Advantage members to identify health needs and close care gaps. They will collaborate with primary care providers and educate patients on disease processes and medications to improve overall health outcomes.
Requirements
Candidates must hold an active, unrestricted license and relevant certification (ANCC/AANP for NPs or NCCPA for PAs) in the state of practice. A valid driver's license and access to reliable transportation are required to perform field-based home visits.
