RN Case Manager

Key Responsibilities

The RN Case Manager creates and monitors care plans across the continuum while advocating for patient resources and removing barriers to care. They also maintain communication with health providers to ensure effective implementation of treatment plans and accurate medical documentation.

Requirements

Candidates must hold a valid Registered Nurse license from the Florida Board of Nursing or a compact state. A diploma from an accredited nursing school is required, and BLS certification is preferred.
