Key Responsibilities
The RN Case Manager performs comprehensive physical and psychosocial assessments to develop and implement individualized patient care plans. They collaborate with the interdisciplinary team and physicians to ensure high-quality, compliant care while providing on-call support.
Requirements
Candidates must hold a current State Registered Nurse license and have graduated from an accredited nursing program. At least two years of clinical experience is preferred, along with a valid driver's license and reliable transportation.
