Medical Director / Primary Care Physician (MD/DO) – Post-Acute Care

Legacy Care is seeking a compassionate, clinically strong physician to serve as Medical Director and attending physician for our Charlottesville-area post-acute care practice. This is an opportunity to combine direct patient care with physician leadership while maintaining greater flexibility and autonomy than a traditional office-based schedule.
You will care for medically complex adult and geriatric patients in skilled nursing, rehabilitation, long-term care and assisted living settings while partnering with advanced practice providers, facility leadership and interdisciplinary care teams.

What You’ll Do
Serve as Medical Director for assigned Charlottesville-area facilities
Provide onsite primary, acute and chronic care to post-acute and long-term care residents
Collaborate with and provide clinical leadership to nurse practitioners and other members of the care team
Partner with facility administrators, nursing leadership, therapy teams, patients and families
Support quality improvement, regulatory compliance and appropriate utilization initiatives
Review clinical concerns, changes in condition and complex cases
Complete timely, accurate documentation and maintain strong communication with facility partners
Promote person-centered goals of care and smooth transitions across the healthcare continuum
What We’re Looking For
MD or DO with an active, unrestricted Virginia medical license
Active DEA registration
Board certification or board eligibility in Internal Medicine, Family Medicine or Geriatric Medicine
Experience in skilled nursing, long-term care, geriatrics, hospital medicine, primary care or post-acute medicine preferred
Medical Director or clinical leadership experience is helpful but not required
Strong communication, collaboration and clinical decision-making skills
Ability to travel locally among assigned Charlottesville-area facilities
Schedule and Compensation
Schedule: Monday-Friday, day rounding
Clinical compensation: wRVU
Medical Director stipend: Monthly, also included is collaboration stipend
Employment structure: Independent Contractor
Anticipated start: As soon as credentialing is completed
