Key Responsibilities
The Unit Based Pharmacist integrates into the interdisciplinary team to manage medication reconciliation, patient counseling, and treatment planning. They are responsible for validating medication orders, monitoring patient profiles, and facilitating safe discharges to improve patient outcomes.
Requirements
Candidates must hold a Bachelor's degree in Pharmacy and a valid Registered Pharmacist license. A minimum of one year of hospital pharmacy experience and Advanced Cardiac Life Support certification are preferred.
