Position: Care Coordinator MSW
Shift: Day Shift; 12 hour shifts plus every 3rd weekend
Experience
1. At least 5 years of recent clinical experience in a physician’s office, clinic, hospital, or emergency services (EMS) setting, or other equivalent experience.
2. Leadership experience a plus.
Education
Master’s Degree in Social Work from CSWE (Council on Social Work Education) accredited program.
Licensures/Certifications
Licensed Social Worker strongly preferred.
Key Responsibilities
Coordinate patient care across the healthcare continuum, including conducting psychosocial assessments and interventions. Collaborate with physicians, colleagues, and community partners to improve patient care and services, support population health, and promote patient-centered care.
Requirements
Requires a master's degree in social work from a CSWE-accredited program and at least five years of recent clinical experience in a physician's office, clinic, hospital, emergency services, or an equivalent setting. A Licensed Social Worker credential is strongly preferred, and leadership experience is a plus.
