Key Responsibilities

The Medical Social Worker provides psychosocial assessments, diagnostic evaluations, and patient-centered interventions as part of an interdisciplinary home health team. They coordinate care, facilitate discharge planning, and advocate for patients while ensuring compliance with regulatory and HIPAA standards.

Requirements

Candidates must hold a Master's degree in Social Work and possess a valid South Carolina LMSW or LISW-CP license. At least one year of health care experience is required, along with a valid driver's license and BLS certification.
