Key Responsibilities
The Medical Social Worker will evaluate the psychosocial needs of hospice patients and their families while providing counseling and crisis assistance. They will also collaborate with an interdisciplinary team to develop care plans and advocate for patient preferences and quality of life.
Requirements
Candidates must hold a Bachelor's degree in Social Work and an active Ohio LSW or LISW license. At least one year of healthcare social work experience and a valid driver's license with reliable transportation are required.
