Job Description
Manages cases to enhance the quality of patient management and satisfaction.
Promotes continuity of care and cost effectiveness through integrating functions of case management, utilization review and management, and discharge planning.
HOURS: Friday - Tuesday 8:00 a. m. to 5:00 p. m.

Requirements
Requires an Associate's degree in nursing and a current Texas Registered Nurse License. Candidates should have experience in case management and proficiency with EMR software and Microsoft Office.
