Key Responsibilities
The Registered Nurse Case Manager will assess patients, develop individualized treatment plans, and coordinate care across various settings to support Service Members and their families. They are responsible for maintaining accurate medical documentation, monitoring patient progress, and advocating for the healthcare needs of their patients.
Requirements
Candidates must hold an active, unrestricted Registered Nurse license and possess at least two to three years of nursing experience depending on their degree. Required certifications include Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), and either a Certified Case Manager (CCM) or ANCC certification.
