Join Our Hospice as a Hospice RN Case Manager – Monday-Friday, PSLF Eligible in Columbus, Indiana, United States. Our Hospice is hiring a Registered Nurse Case Manager (RNCM) for the Columbus team serving the Bartholomew and surrounding counties in our service area. This person works with a caseload of patients and the help of Nurse Practitioners, Physicians, Aides, Chaplains, and Social Workers.

Key Responsibilities
The RN Case Manager is responsible for managing a caseload of hospice patients by conducting home visits, performing clinical assessments, and developing individualized plans of care. Requirements
Candidates must hold a current Registered Nurse license in Indiana and possess either an Associate or Bachelor degree in Nursing. The role requires the ability to travel to various patient locations using a personal vehicle, along with a valid driver's license and car insurance.
