How you will contribute

The RN Case Manager is responsible for providing end-of-life care and hospice
services to patients in their home or community setting. This includes
intermittent skilled nursing services and directs, supervises, and instructs
hospice aide staff in the provision of personal patient care. 

This role requires compassion, strong clinical judgment, excellent communication
skills, and the ability to support patients and families through highly
sensitive and meaningful experiences. 

How you will make an impact

• Assume primary responsibility for the patient/family caseload that includes

• Report On-Call activities to the clinical leadership and Branch RN Case

• Maintain and complete appropriate care coordination notes for visit notes, and

• Assist, provide, or arrange for delivery of medications and supplies needed

• As scheduled, attend interdisciplinary team meetings during on-call period for

• Under the physician’s order, admits patients eligible for hospice services in

• Reports patient status and need for other disciplines to clinical leadership,

• Develops patient care plan that specifically addresses identified patient

• Be responsible to ensure the use of the 4Ms (What Matters to the patient,

The Experience and Skills You’ll Bring

* Current unrestricted RN licensure in the state of practice 
* Valid driver’s license and proof of auto insurance
* Two years of RN experience 
* Two years of Hospice/Palliative Nursing experience preferred

Schedule: Mon-Fri 8am-5pm. On call as needed.
